Families new to School District 21 receive these forms when they arrive at the school office to register their child/children. Parents are welcome to print these forms in advance and complete them prior to arriving at school to register.

Title I Compacts
This compact outlines how the parents, the entire school staff, and the students will share the responsibilities for improved student academic achievement and how the school and parents will build and develop a partnership to help the children achieve state standards.
Erin’s Law
The Comprehensive Health Education Act often referred to as “Erin’s Law”, was passed in the Illinois state legislature on January 24, 2013. This law requires that age-appropriate sexual abuse and assault prevention education is included in grades pre-kindergarten through 12.
Internet Safety
As technology continues to evolve, people are more and more connected to the Internet–through computers, mobile telephones, video games, and soon, even kitchen appliances! While the Internet provides all types of useful information, resources, and activities for individuals, families, and organizations, like venturing out in the world, there are also risks associated with its use. Information about teaching ourselves and our children good habits with technology can be found here in English or Spanish.
MealTime Online
MealTime Online allows you to create an online account in order to prepay deposits and payments on balances for your student’s school lunch account, conveniently, safely, and securely. The above link provides directions on how to set up an account.
